About Us

Incarceration does not reduce a person's humanity

 Midwest Prohealth is a Minnesota 501(c)(3) public charity headquartered in Rochester, Minnesota. We are developing an evidence-anchored, non-clinical chronic disease education, self-monitoring, and care continuity model for people in correctional custody and those returning to their communities. Our work begins with respiratory disease in Minnesota and is designed for wider adoption across the Midwest and comparable underserved communities nationally. 


Our mission is to ensure that leaving incarceration never means leaving care: continuous, dignified health support through education, care continuity navigation, and community partnership. 


WHY WE STARTED

The gap is predictable. that means it's preventable

The weeks after release are among the most medically dangerous. People leave with chronic conditions and no coverage, prescriptions and no refills, and no providers. None of this is a surprise; it happens to hundreds of thousands of people a year. We close that gap, with prevention, coordination and follow-through.
