Midwest ProHealth Initiative is a nonprofit public health organization rooted in the Midwest and working nationally. We exist because the health care system routinely loses people at the moment of release and the cost is measured in lives.
Our mission is simple to state and hard to do, to make sure that leaving incarceration never means leaving care. We do it with evidence-based programs, deep partnership with correctional systems and community providers, and an unshakeable commitment to dignity.
The weeks after release are among the most medically dangerous. People leave with chronic conditions and no coverage, prescriptions and no refills, and no providers. None of this is a surprise; it happens to hundreds of thousands of people a year. We close that gap, with prevention, coordination and follow-through.
Health education and access to care are rights, not privileges, regardless of incarceration history.
Every person is met with respect and without judgment, at every point of contact.
We build relationships before programming, and we work through the communities people return to.
